Visiting a new dentist’s office shouldn’t be a stressful and uncomfortable experience. We want you to feel prepared and at-ease when you walk through our doors instead of being bombarded with paperwork. To make sure that you feel at-ease and welcome, we’ve compiled some documents and helped you know what to expect during this first visit. If you have any questions or would like to learn more, don’t hesitate to reach out to our staff before your appointment.
We encourage all of our patients to bring in completed new patient forms to their first visit to help save them from needing to fill them out in our office. As soon as we input your personal and insurance information, we’ll guide you back to one of our treatment rooms for your appointment. We’ll begin by taking any necessary X-rays and conducting a thorough examination of your teeth and gums. Once complete, we’ll remove all plaque and tartar from your teeth, as far down as the gumline. Before you leave, we’ll polish and floss between each of your teeth, leaving your smile feeling fresh and healthy.
To help make your first appointment faster and less stressful, we’ve included a link to fill out our patient forms below. That way, instead of needing to sit in our office and take 15 minutes to fill them out and scramble for your insurance card and other personal details you’ll need to fill in, all we’ll need to do is input the information you’ve already provided us with. If you have any questions or aren’t able to access the forms on the link below, reach out to us!

Certain procedures, like porcelain veneers or teeth whitening, aren’t always covered by dental insurance. To allow you to receive these procedures without having to worry about paying everything out-of-pocket after your appointment, we also offer dental financing. This allows you to make manageable monthly payments with little to no interest for your convenience. To learn more about financing options, contact us today.
Our patients who don’t have dental insurance or who are looking to save big on certain treatments, we also offer an in-house savings plan. With this option, you won’t have to worry about monthly or annual premiums, paying deductibles, or annual maximums. All you’ll need to do is pay the yearly membership fee and you can start enjoying discounts on services. To learn more details about our plan, ask one of our front desk staff members!
